Your pre-admission information needs to be completed and returned to the Mater Hospital at least 1 week prior to your admission (or as soon as possible if your procedure is in less than 1 week). The following options are available for completing your admission form:

Option 1: You can submit your pre-admission information online through the Mater's pre-admission portal, completing your admission paperwork through the portal will allow you to store your information for future admissions.

Option 2: You can download the Mater Pre-Admission Form and save it to your computer, you will then be able to electronically complete the form. Once complete, save the form to your computer again then return to the Mater Hospital by fax, email, post or in person.

Option 3: You can print and complete the Mater Pre-Admission Form and return it to the Mater Hospital by fax, email, post or in person.
